8-9 сентября 2010 года во Владивостоке проходила Межрегиональная научно-практическая конференция на тему «Современная микробиологическая диагностика туберкулеза» для специалистов противотуберкулезных учреждений Дальневосточного и Сибирского Федеральных округов. Инициатором проведения конференции выступило ГУЗ "Приморский краевой противотуберкулёзный диспансер". В работе конференции приняли участие 70 специалистов противотуберкулезных учреждений, в т.ч. 58 представителей противотуберкулезных диспансеров Дальневосточного и Сибирского федеральных округов (заведующие бактериологическими лабораториями, заместители главных врачей по лечебной работе), 7 ведущих научных сотрудников российских НИИ фтизиопульмонологии, ФСВОК и 5 представителей научных объединений по производству диагностических систем для культуральной и молекулярно-генетической диагностики туберкулеза. На конференции обсуждались проблемы повышения качества микробиологической диагностики туберкулеза, опыт взаимодействия специалистов противотуберкулезной службы и учреждений общей лечебной сети, сотрудничества клиницистов и специалистов лаборатории. Была поднята тематика обеспечения внутреннего и внешнего контроля качества исследований, направленного на улучшение качества лабораторной диагностики туберкулеза в регионах. Рассмотрены вопросы совершенствования методов обнаружения, идентификации, определения лекарственной чувствительности возбудителей туберкулеза. Представлены результаты применения ускоренных культуральных методов, молекулярно-генетических и иммунологических методов в диагностике туберкулеза, рассмотрены рекомендации ВОЗ. Участники конференции ознакомились с передовым опытом в диагностике и лечении туберкулеза с множественной лекарственной устойчивостью возбудителя. Особый интерес представляли доклады об опыте противотуберкулезных учреждений при работе с лабораторными информационными системами и автоматизации деятельности лабораторий. Живую дискуссию вызвали сообщения из регионов о возникающих проблемах при работе с помощью автоматизированных систем. Представленное одним из участников конференции решение проблемы обеспечения бесперебойного энергоснабжения медицинского оборудования с помощью специально разработанной системы требует дальнейшего оформления изобретения и широкого внедрения в медицинские учреждения. По материалам, представленным на конференции подготовлен сборник тезисов; принято решение совместно с НИИ фтизиопульмонологии ММА им. И.М. Сеченова разработать методические рекомендации для специалистов противотуберкулезных диспансеров и учебных медицинских учреждений «Современная микробиологическая диагностика туберкулеза». Живой отклик у всех участников конференции вызвала идея создания специального сайта для микробиологов противотуберкулезной службы для живого общения, обмена опытом, информацией, научно-практической литературой, нормативными документами.

On July 6-7, 2010, the Russian Health Care Foundation will hold a conference to discuss the most important issues related to and aspects of TB+HIV co-infection, under the Program: Promoting a Strategic Response to TB -- Treatment and Care for Vulnerable Populations in the Russian Federation. The Conference will take place in Moscow.
Though initially, it was intended to bring together specialists from the Central Federal District, we are glad to invite all specialists who are willing to participate. The RHCF will cover the board and lodging expenses of participants (during the Conference), while travel expenses are to be assumed by participants themselves.

Infectionists and TB doctors from four Russian regions (the Volgograd, Ulyanovsk, Kemerovo and Ivanovo Oblasts) shared their experience in cooperating with non-profit organisations actively working with target groups.
A particularly important issue under the discussion was related to discrepancies in reporting on co-infection cases (TB+HIV), using different forms for federal statistical observations - Form # 33 and Form # 61, as well as causes to defects in communication of information about patients screened for TB and HIV between AIDS Centres and TB dispensaries. Participants of the roundtable decided to send a letter to the MOHSD to request it to provide clarifications of the wordings in the reporting forms and specify the information sharing arrangements.
They also discussed monitoring of the R4 Program funded by the GF. Grant proceeds were spent to establish reference diagnosis and HIV pre- and post-testing counselling offices in TB Dispensaries. They have been actively operating for 2.5 years; however, the statistical reporting of cases is yet not well-established. Unfortunately, specialists from the CPHRI could not join this discussion.
Apart from health workers, active participants of the roundtable included representatives from the GBC (Global Business Coalition against HIV/AIDS, TB and Malaria), and specialists from the University Research Company (USAID), who shared their experience in screening of HIV patients for TB in St-Petersburg.

As part of the Program: Promoting a Strategic Response to TB -- Treatment and Care for Vulnerable Populations in the Russian Federation, the MOHSD’s Centre of TB Care for HIV Patients will hold the 3rd Russian National Conference (with international inputs) on Multi-Drug Resistant TB in HIV Patients.

The Conference will take place in Moscow on May 12-13, 2009. Board and lodging costs of its participants will be paid by the Russian Health Care Foundation.

TB+HIV care specialists/coordinators from Russian regions and managers of treatment and diagnostic services from AIDS and Communicable Disease Control Centres are invited to take part in the Conference (no more than 2 persons from a region).

Proceedings of the Conference will be published in the 7th issue of the TB/HIV Epidemiological Bulletin in 999 copies for all participants of the Conference.

Закончена реализация проекта создания Обучающего центра по организации выявления, диагностики и лечения туберкулеза, включая туберкулез с множественной лекарственной устойчивостью при УФСИН России по Томской области на базе учреждения ФБУ ЛИУ-1. Учебный центр организован при поддержке, Министерства здравоохранения и социального развития Российской Федерации, Центрального НИИ туберкулеза РАМН, ФСИН России. Для обучения слушателей Обучающего центра было закупленно демонстрационное, компьютерное, диагностическое оборудование, оборудование для проведения видео конференций и мебель, было выделено и отремонтировано помещение площадью 100 кв. метров для оборудования лекционной аудитории.
С июня 2008 года в Обучающем Центре проводятся обучающие семинары с врачами и медицинскими работниками, в том числе и из стран ближнего зарубежья. На 01.12.2008 года Медицинским отделом УФСИН России по Томской области было обучено 48 врачей и 70 средних медицинских работников.

Заключен Договор с ГУ ЦНИИТ РАМН на проведение серии семинаров по теме «Повышение квалификации лабораторных работников противотуберкулезных учреждений в области выявления туберкулеза ускоренным культуральным методом с использованием автоматизированной системы BACTEC MGIT 960». Четыре пятидневных семинара пройдут на базе ГУ ЦНИИТ РАМН в период с 17 ноября по 12 декабря 2008 года. Всего будет обучено 60 лабораторных работников.
В процессе обучения учащиеся получат необходимые знания о методике обработки и посева диагностического материала на жидкие среды, культивированию, приемам идентификации возбудителя, принципам постановки теста определения лекарственной чувствительности к препаратам I и II ряда и регистрации и учета результатов с использованием программного обеспечения EpiCenter для автоматизированной системы BACTEC MGIT 960.

Russian Health Care Foundation announces prolongation of the tender among organization on development of a TB distance learning system within the frames of Program “Promoting a strategic response to TB treatment and care for vulnerable populations in the Russian Federation”.

Within the limits of tender 1 organizations will be selected and grant agreement will be signed for the period of August - September 2007. A system may be based both on widely used in Russian organisations international programmes and specific Russian specially developed programmes.
Grant will be used for payment of:
- Remuneration of specialists: programme makers, TB doctors, teachers, etc.
- Procurement / development of programme.

During June 5th till July 25th, 2007 the Russian Health Care Foundation will accept applications from the Russian organizations. Application must contain an official letter addressed namely Mr. Goliaev Dmitry Arkadievitch, Global Fund Project Director, a copy of Charter of the organization, Statement from Tax Inspection about absence of debts, copies of certificates aboiut registration in Tax Inspection and State registration.
Pre-qualification will be held within 26-31 July 2007.

Within the GF Program for 2006 a conduction of 213 short-term trainings was projected for medical workers of TB settings and more than 4000 participants will get trainings.

As on the end of August 2006 the following trainings were conducted:

Subject “Promotion of infectious control standards in TB settings” within the frames of 10 days workshop[, organized together with the Charity Organization “Partners to the name of health” at Novosibirsk TB Research institute; 30 participants were trained – officers of profile facilities of civilian and penitentiary sectors from Amursk, Irkutsk, Kemerovo, Chita, Omsk, Tomsk, Novosibirsk Oblasts as well as from Ust’-Ordynsk Buriatsk, Aginsk-Buriatsk, Taimyr autonomic Oblasts, and from Khakassia , Altay, Sakha, Tyva, Buryatia Republics, Khabarovsk and Krasnoyarsk Districts;

Subject “General Management and Logistics”. 2 trainings were conducted, 50 participants received training – officers from civilian and penitentiary sectors of Volgograd, Kostroma, Voronezh, Lipetsk, Astrakhan’, Saratov, Orel, Penza and other Oblasts, as well as of Dagestan, Kalmykia, Mordovia, Ingushetia Republics. Professor Gilbert Kombe, MD, MPH, Senior HIV/AIDS Technical Advisor, the Partnership for Health Reformplus Project Abt Associates Inc was contracted as trainer. Trainings were conducted at Russian Academy of Medicine of post-graduate education;;

Subject “Drugs Management”. 14 trainings were conducted (1 – at federal level, 3 – at District level, 10 – of Oblast level), 398 participants received training.

Subject “TB treatment and Monitoring”. 6 trainings were conducted, 184 participants received training. Trainings were conducted for TB doctors from Republics of Mariy-El, Dagestan, Komi, Buryatia, Kurgan and Tambov Oblasts.

Subject “Cultural researches and detection of drugs vulnerability”. 3 trainings were conducted, 45 participants received training. Two trainings were conducted at the Central TB Research Institute of Russian Academy of Medical Scientists for the specialists from Central and South Federal Districts and one training was conducted at Kemerovo Oblast TB dispensary for laboratory specialists from Siberia and Far East federal Districts. Materials prepared with the participation of WHO have been used as education materials.

Till the end of 2006 the following activities were projected: 5 workshops designed for “General Management and Logistics”; 1 workshop designed for “TB Treatment and Monitoring” for specialists from Karachaevo-Cherkessia Republic; 26 workshops (2 – at federal level, 4 –at District level and 20 – at Oblast level); subject “Drugs Management on TB drugs”; 3 workshops designed for “Cultural researches and detection of drugs vulnerability”, 9 workshops designed for «MDR-TB treatment” for doctors and nurses at MDR-TB Excellence Centers of Federal Penitentiary Sector and profile research institutes; more than 100 seminars on TB/HIV problems; 2 workshops on arrangement of social support to TB patients in accordance with WHO’s pattern (18-19 and 25-26 September, 2006, will take place in Orel and Tomsk).

In the framework of the TB Programme of the Global Fund Project there will be conducted two two-days training seminars on Management and logistics for 25 Russian specialists each. Seminars will take part in Moscow on 26-27 of June and 29-30 of June with participation of international expert and trainers from the Russian Academy of post-diploma Education.
